## Begin with the safer home choices

Ease back from the motion that causes soreness. Then add gentle movement as the joint settles. Your doctor may suggest a brace or medicine that fits your health.

These steps aren't hoops to clear before other care. They may help you sleep, walk, or use the joint more easily. If daily use stays hard, an exam is a sound next step.

## Keep home care steady enough to judge

Stay with an easy exercise plan instead of changing it daily. Tell the therapist which movement hurts and when it eases. You'll adjust the amount without stopping all activity.

Write down every medicine you take before a visit. Anti-inflammatory medicines, such as ibuprofen or naproxen, lower soreness and swelling. Blood thinners, aspirin, and those medicines may affect whether care can go ahead. Don't stop prescribed medicine unless the doctor managing it agrees.

## Get prompt care for warning signs

A warm, swollen joint plus fever needs quick care. A fresh injury that keeps you from standing also needs attention. Spreading redness or drainage after joint care can't wait.

Sudden weakness, lost feeling, or lost bladder control needs urgent help. If a loud pop leaves you weaker, a tendon may be damaged. These problems can't wait for a planned PRP appointment.

## Get every cost and side effect in writing

PRP is an abbreviation for platelet-rich plasma. A provider draws and spins your blood, keeping platelets, blood cells used in clotting and early repair. The American Academy of Physical Medicine and Rehabilitation convened a technical expert panel that ran a structured literature review (2023, updated through June 2025) and a modified Delphi process, and issued five evidence-based clinical recommendations plus 11 consensus-based best practices for PRP in knee osteoarthritis. The statement is explicit that orthobiologic therapies 'remain an evolving area of practice' and that robust, dose-dependent randomized controlled trials are still needed to establish PRP's clinical effects.

The 2019 ACR/Arthritis Foundation osteoarthritis guideline makes STRONG recommendations for exercise, weight loss in people with overweight or obesity, self-management programmes, tai chi, cane use, tibiofemoral bracing, topical and oral NSAIDs and intra-articular glucocorticoid injections for knee OA. Its conditional recommendations cover balance exercises, yoga, CBT, acupuncture, thermal modalities, radiofrequency ablation, acetaminophen, duloxetine and tramadol. Anything offered before a course of the strongly recommended options is being offered out of order.

A Bayesian network meta-analysis of nine studies (six RCTs, 1055 patients) found leukocyte-POOR PRP produced significantly better WOMAC scores than hyaluronic acid (mean difference -21.14; 95% CI -39.63 to -2.65) and than placebo (-17.84; 95% CI -34.95 to -0.73), while leukocyte-RICH PRP showed no such significant difference versus placebo. PRP of either type caused more local adverse reactions than hyaluronic acid (OR 5.63; 95% CI 1.38-22.90), almost always local swelling and pain, with no difference in safety between the two PRP types.

Medicare's national coverage policy covers autologous platelet-rich plasma ONLY for patients with chronic non-healing diabetic, pressure and/or venous wounds, and only within an approved coverage-with-evidence-development clinical study. There is no Medicare national coverage for PRP in osteoarthritis or tendinopathy, which is why these injections are billed to the patient as cash-pay.

The devices used to spin PRP at the point of care are cleared by FDA as clinical centrifuges, product code JQC, a Class I device under 21 CFR 862.2050 - for example the Biomet GPS Platelet Separation Kit (K030555, cleared 2003) and the Harvest SmartPrep2 / SmartPrep Platelet Concentration System (K103340, cleared 2010). That clearance covers the equipment that separates blood. It is not an FDA approval of platelet-rich plasma as a treatment for osteoarthritis, tendinopathy or any other orthopedic condition, and copy must never blur the two.

PATH-2 randomised 230 adults with acute Achilles tendon rupture managed non-surgically to PRP or a placebo dry-needle injection across 19 UK hospitals, with a central laboratory confirming the PRP was of good quality with the expected growth-factor content. At 24 weeks there was no detectable difference in muscle-tendon function (limb symmetry index 34.7% versus 38.5%; adjusted mean difference -3.9%; 95% CI -10.5% to 2.7%) or in any secondary outcome or adverse-event rate.
